Sports physicals are often required by middle and high school-sponsored teams or private teams. Often these visits can be done at your child's annual well visit depending on the date of your child's last check up. It is important to let us know your child will need a sports physical when you make your child's appointment.

The VHSL sports physical form (click here) is required for each school year. You can print this form and bring it to your child's visit. Please complete your portion of the forms before your visit. Everything except for page 3 (Medical Eligibility) and page 6 (Physical Examination) are for the student and parent/guardian to fill out. The student and parent/guardian must sign the form as well.

Insurance may cover sports physicals. All Virginia Medicaid Managed Care plans do. Ask about the cost when you are making the appointment.
