Accepted Plans

Co-payments

Your co-payment is determined by your individual insurance plan. You will be expected to pay your co-payment at the time of service. If you receive a bill after paying your co-pay, your insurance has processed the claim and deemed that you owe an additional amount for the services you received in our office. This may cover such services as lab fees, therapeutic injections, nebulizer treatments, urine tests or other tests not covered by your insurance policy.

Billing Policies

Claims are usually processed by your insurance company within two to four weeks after your visit. If a balance remains due after our claim has been processed, we will bill you for that amount. Bills for a balance $25 or more are mailed on a monthly basis. Bills for balances under $25 are mailed on a quarterly basis. Please review the Explanation of Benefits (EOB) sent by your insurance company and call our office to pay the balance due. We gladly accept credit card payments over the phone.

Red Flag Policy

We are now required by law to request photo identification if you have not been seen in the last six months. This new policy is to protect you from identity theft. If your current address does not match the address on your identification, please bring a utility bill to verify your current address.

Notification of New Insurance

You should bring your child’s insurance card to each visit so that we can verify coverage. If your coverage has changed, we will ask to copy the insurance card for our files.

Change of Address and/or Contact Number

We ask that you provide our office with the most up-to-date home address and home, work and mobile phone numbers. It’s critical that we have this information so that we can contact you with medical information. We would like for you to provide your email contact information and indicate whether you would like to receive text message reminders.
